Overview

David is Emeritus Professor of Mental Health Services Research at the University of Exeter, an emeritus Senior Investigator in the UK NIHR Faculty and until recently the inaugural Head of Nursing at Exeter which took its first students in 2019.  He has been a registered nurse for almost 40 years and a health services researcher since 1997. He undertakes pragmatic mixed methods studies, combining experimental and observational research on complex interventions, to significantly improve the usefulness of research for clinicians and patients.

After nursing auxiliary, student and staff nurse experiences David took an 18 month post-registration qualification in nurse behaviour therapy at the Institute of Psychiatry and Maudsley Hospital London, specialising in delivering treatments for people with anxiety problems. After a number of posts, including as a research assistant and a nurse tutor, he studied first for a bachelors degree (this being well before nursing was a graduate profession) and then a PhD. 

The National Institute for Health Research (NIHR) has been fundamental to David’s career, having funded  much of his research, including improving access to healthcare for people with common mental health disorders such as anxiety and depression. His most significant contribution to healthcare in the UK was as part of the team that established the Improving Access to Psychological Therapies (IAPT) programme at the Department of Health from its inception in 2005, regarded as second only in importance to the shutting down of the Victorian asylums in terms of significant mental health reform. David was an external advisor to the IAPT programme from 2008-2011. David pushed for ‘low-intensity’ simple interventions to be included in IAPT as a fundamental part of the operating principles and established the base for a completely new para-professional ‘Psychological Wellbeing Practitioner’ workforce – now numbering over 3,000 in England alone – including the role definitions, curricula and training courses throughout the NHS. The research activities that delivered the evidence base for this development had been facilitated by NIHR funded awards and formed a 4* impact case study in the Research Excellence Framework.

More recently, although in much the same vein, David has been undertaking clinical trials of simple psychological therapies applied to depression, culminating in one the most highly cited healthcare publications of 2016 which reported the results of his 'COBRA' trial in The Lancet. He has also investigated nursing roles in primary care telephone triage, a research programme he first initiated in 1999. His emphasis in all this work is that people should receive rapid access to healthcare that is efficient, effective and equitable.

His current research activity – also funded by NIHR – is a programme of research to develop, test and evaluate  essential nursing care. This was initially through the  ‘ESSENCE’ programme, looking to respond to increasing criticisms of nursing, including those in the Francis Report, that nurses have neglected their traditional caring role. He is leading an international group of nurse scientists who believe that good research can deliver the much needed evidence to directly address these criticisms and rediscover the traditional focus of nursing – hands on caring for patients. Findings from David's ESSENCE research programme are at the heart of nursing education in University of Exeter Academy of Nursing. Even more recently, David was chief investigator of the 2020-22 'COVID-NURSE' cluster randomised controlled trial which developed, and evaluated, a pandemic-specific fundamental nursing care guideline for patients admitted to hospital with the SARS-COV-2 viral infection.

David was also President of the European Academy of Nursing Science (EANS) from 2012-2018. He led both EANS and a collaborative group of nurses and other health care practitioners on a mission to improve the quality and relevance of research – the REFLECTION network. As the editor of the world’s first ever comprehensive textbook on complex interventions research methods, published in 2015 he is at the forefront of both clinical and methodological developments in health services research. 

David's work was recognised by the NIHR when it awarded him the status of ‘Senior Investigator’ – one of very few nurses in the group of researchers regarded by NIHR as the top 200 in England. After eight years as a senior investigator he is now 'Emeritus'. As part of his continuing committment to the NIHR, he leads the research masterclass programme for early career and aspiring researchers in nursing and midwifery, part of the NIHR Nursing and Midwifery Incubator. Over 600 nurses and midwives have applied to join these masterclasses.

Whilst there is much still to be done, David never forgets that research activity and its findings should always provide clinicians on the ground with information about how they can work to deliver the best possible clinical care to their patients. It is this key principle that drives him.

Teaching

David led the development of the University of Exeter Academy of Nursing which took its first undergraduate students in 2019. Until September 2016, David was programme lead for the University of Exeter Medical School's Masters in Applied Health Services Research which is structured around programmatic, mixed and sequential research methods for developing and researching complex health care interventions. David also wrote the UK national curriculum for the new national workforce of Psychological Wellbeing Practitioners (PWPs) working within the Department of Health's Improving Access to Psychological Therapies (IAPT) programme. He has published the Department of Health's commissioned educator and student support materials with his colleague Mark Whyte from the University of York, now in their third edition and has co-edited two major textbooks: 'The Oxford Guide to Low-intensity CBT' and 'Complex Interventions in Health: An Overview of Research Methods'.
